I know there are many different programs out there to help you set up a private practice some are very expensive, and you don’t know if they will work or not.

The Book Therapist Practice in a Box: The Ultimate guide to building a Successful Private Practice. This book takes you from your idea to a working private practice. It breaks down all the to do’s into easy actionable steps.

Forms 12 in all: these are all the forms your need to start a private practice today. All you must do is customize them for your practice. Easy to do and takes about 10 minutes.

Assessments: Intake forms/social history, presenting problems and symptoms, mental status, drugs, alcohol suicidality/homicidally, genogram, etc. (Initial intake interview.)

Case consult/personal notes: This can be used for psychotherapy notes, billing, collaterals, follow-up questions.

Client Data: (registration) This form has client demographics and the necessary information to bill insurance. We also collect referral sources, email addresses, and who to contact in case of an emergency (think Tarasoff v Regents of the University of California “Duty to Warn”’ and Ewing v. Goldstein. The court expanded the definition of Civil Code § 43.92 to "include family members can provide information to the therapist.” (California, 2004)

Credit Card: A form to store credit card information giving you the ability to charge for appointment copays, session fees, or missed appointments.

Fees statement: Clearly spell out what you charge for each service. Be clear and include items like letter writing, additional phone calls between sessions, court costs, etc. These can also go into your informed consent, or as I use it could a separate document.

HIPAA: Notice of Information (HIPAA): Spells out to your clients what you are doing to protect their privacy.

Informed consent for Minors 12-18: This is specific to minors and is broken down as being easily readable as required by law.

Missed or late cancel appointment letter: Think about your time, set a boundary and stick to it.

Office Policies & adult Informed Consent: General office policies and consents. Include confidentiality and exceptions, hours, emergency contacts, and cancellation time frames.

Progress Note: Weekly comprehensive case note.

Release of Information: Provides permission to share information. Even to write a letter on behalf of your client you must obtain a sign authorization from them.

Social Media Policy: It’s good to let clients know what your policy on social media; no friending or following allowed.

Doctor’s Introduction letter: The letter to inform doctors or other professionals about your practice.

Missed appointment follow-up client letter reminder (bonus): Okay, clients will miss sometimes, this letter will provide a reminder of your policy.

Welcome Letter: We use this to provide an introduction to us and our practice. Since we are the owners of this group we want to provide great service, but we also want our clients to know we care, and give them an easy way to contact us. I provide my cell number and ask that the client call me if they have any concern or problems. We want them to be happy as possible with the process. In seven years, we have had only two clients change therapists due to “just not a good fit.” Since I started providing my personal cell number, I have only received one call after hours. I think this one feature is only part of what sets us apart from the crowd. You could use this as a sole practitioner, but it’s most effective in a multiple therapist office. We also use this a marketing tool for great customer service.
